Program Description

A.F.C. provides in home health care to all candidates wanting in home care. We have skilled nursing, LPN’s, skilled aides, caregivers, transportation, etc. We encourage choice, independence individuality, and growth. A.F.C. delivers clinically excellent, compassionate care to patient where they are most comfortable – at home.
